How do i apply for the child benefit tax and universal health care tax? b.c?

my baby is 7 weeks im wondering how i apply for these the website said i should have the application with my birth registration package..? i dont remember seeing this did i miss something?How do i apply for the child benefit tax and universal health care tax? b.c?I got the registration form from the hospital with all my birth registration papers. I'd call the hospital if I were you and see if they have them, maybe they just forgot to put one in with your registration package.
